🔒 Required Donor Certification (Florida Law)

⚠️ IMPORTANT: All donors must complete this certification before donating. This protects both donors and the foundation.

Florida law requires that we obtain certification from all donors. By making a donation to TAD Charitable Foundation, donors certify the following:

I am NOT a "foreign source of concern" I am not affiliated with governments, officials, political parties, or entities from: China, Russia, Iran, North Korea, Cuba, Venezuela (Maduro regime), or Syria.
This donation is from lawful sources This contribution is not made on behalf of any prohibited foreign source or entity controlled (25%+ ownership) by a foreign source of concern.
I understand the legal requirements I understand that providing false certification may result in legal consequences under Florida and federal law.

Why this certification is required: Florida Senate Bill 700 requires all charitable organizations to obtain certification that donations do not come from foreign sources of concern. This protects Florida charities, ensures transparency, and safeguards against foreign interference.
